Mom's Heart

Mom's Heart is a boss that can appear on the second floor of Chapter 4 (Womb, Utero), and is the final boss of Endings 1-11. • Prior to Booster Pack #5, it could also appear in the Scarred Womb. After being killed 11 times, Mom's Heart is permanently replaced by It Lives. However, even after being replaced by It Lives, there are a few circumstances in which Mom's Heart can still be fought: • It can still appear in the Nightmare Boss wave of Chapter 4 in Greed/Greedier Mode. • It has a chance to appear in one of the boss rooms in The Void. • After the defeat of the stronger version of Mom on the second floor of Chapter 3.5 (Mausoleum, Gehenna), a flesh door appears, which can be opened if Isaac has collected both Knife Piece 1 and Knife Piece 2. • Every time Mom's Heart is defeated changes the current savefile's image until It Lives is unlocked.
• Until It Lives is unlocked, Mom's Heart cannot be continued past unless a Devil Room or an Angel Room appears, which allows access to Sheol or the Cathedral, respectively.
• Just like Mom, using The Bible or XV - The Devil? against Mom's Heart will kill it instantly.
• If Forget Me Now or a 5 pip Dice Room is used on Womb II/Utero II/ Scarred Womb II after defeating Mom's Heart, and Mom's Heart is again defeated on the regenerated floor, it will count as having defeated Mom's Heart 2 separate times, therefore unlocking 2 consequent endings.
• Erasing Mausoleum/Gehenna Mom's Heart with the Eraser will not open the passage to Corpse.

• Mom's Heart appeared in the original The Binding of Isaac.
• In the original Binding Of Isaac, the only attack Mom's Heart could perform was spawning 2-4 monsters before disappearing and repeating the same move each time the wave of monsters were killed off.
• The music that plays during this fight is titled Ventricide. Ventricide is a made-up word formed from the word ventricle (one of the two larger chambers of the heart) and the suffix -cide, which denotes an act of killing. This theme is used in other tracks, such as Matricide, Hericide, and Infanticide.
• Ventricide could also be a junction of the prefix ventr- (from the latin word venter), which could denote "belly", "stomach", "womb" or "fetus", with the suffix -cide.
• Mom's Heart shares this theme with It Lives.
• If Isaac takes damage from the Brimstone attack, Mom's laugh can be heard.
• As of Booster Pack #5, since the Scarred Womb can only be unlocked after defeating It Lives 30 times, Mom's Heart can no longer appear in that environment.
• During the versus screen, Mom's Heart's enemy-spawning sound can be heard. Mom's Heart is one of seven/ six bosses that can be heard during the versus screen, the other six/ five being The Stain, The Forsaken, Big Horn, Mom, It Lives, and Mother.
• The floor graphic beneath Isaac and Mom's Heart in the boss VS Screen in Mausoleum/Gehenna is of a slightly lighter shade of red than in the Womb.
• In addition to this, despite having a Womb backdrop, Mom's Heart's room in the Mausoleum/Gehenna does not have the red static effect of the Womb.
• Getting killed by the final ring of projectiles released after killing Mom's Heart will play the respective ending if there is one, and will count as a Mom's Heart kill. However, it is still considered a loss and will not count towards a win streak.
• Originally in Antibirth, the Mausoleum version of Mom's Heart could spawn enemies like Bouncers and Blurbs.
• Isaac consistently killing Mom's Heart, followed by the first appearances of the Ending Chest, could represent how his death hurt his mother. Considering how in the Final Ending after defeating The Beast, Isaac's Mom can be seen heartbroken about her son's demise.
