Tainted Eve is a character added in The Binding of Isaac: Repentance. She is the alternate version of Eve with ragged hair, no bird, ruined mascara as if crying, and an occasional cosmetic blood trail. Tainted Eve is unlocked by reaching Home and using the Red Key, Cracked Key, or Soul of Cain to access a special room in one of the walls while playing as Eve. Tainted Eve starts with 2 red hearts, and possesses the lowest base fire rate of all characters in the game, even slower than most Familiars. Unlike her normal counterpart, Tainted Eve does not start with Dead Bird or Whore of Babylon. Instead, Tainted Eve starts with Sumptorium in her consumable slot.

• The Empress - Defeat Ultra Greedier as Tainted Eve.
• Soul of Eve - Defeat Hush and Boss Rush as Tainted Eve.
• Horse Pill - Defeat Mega Satan as Tainted Eve.
• Sumptorium - Defeat Delirium as Tainted Eve.
• Lil Clot - Defeat Mother as Tainted Eve.
• Heartbreak - Defeat The Beast as Tainted Eve.

• The achievement for unlocking Tainted Eve is "The Curdled", a possible title for the character.
• To curdle is to "separate or cause to separate into curds or lumps", representing Eve's blood clot familiars.
• A blood clot is known by the game as a "BLOOD_BABY" in the modding API.
• Tainted Eve's card, III - The Empress? means light and truth, a reference to the original sin (consuming the fruit of knowledge of good and evil), but is also associated by some to miscarriage, referenced by the clots.
• This card is a play on the upright III - The Empress which summons the Whore of Babylon effect, her normal starting item.
• Tainted Eve's built in Whore of Babylon effect requirements could be referencing the item of the same name from the original game, where it only activated at half a red heart.

Bug! When Tainted Eve enters a boss room which moves her to a different starting position, such as Mom or Gurdy, the clots will not move with her, potentially leaving both Eve and the clots extremely vulnerable for a moment.
