Azazel is a character unlocked by making three deals with the Devil in one run. Azazel starts with a short-ranged version of Brimstone which deals damage 13 times in one second 9 times in one second, the ability to fly, 0 - The Fool card, and three Black Hearts without any regular Red Heart containers; however, unlike ??? and Dark Judas / Tainted Judas, Azazel can still obtain Red Heart containers with items.

• Begotten Baby - Defeat Mom's Heart on Hard mode as Lazarus Defeat Mom's Heart on Hard mode as Azazel.
• Daemon's Tail - Defeat Satan as Azazel.
• Satanic Bible - Defeat Isaac as Azazel.
• Demon Baby - Defeat The Lamb as Azazel.
• Abaddon - Defeat ??? as Azazel.
• Lilith - Defeat Ultra Greed as Azazel.
• Bat Wing - Defeat Ultra Greedier as Azazel.
• Maw of the Void - Defeat Hush as Azazel.
• Black Baby - Defeat Mega Satan as Azazel.
• Dark Prince's Crown - Defeat Delirium as Azazel.
• Devil's Crown - Defeat Mother as Azazel.
• Lil Abaddon - Defeat The Beast as Azazel.
• The Benighted - Use Red Key (etc.) to open the hidden closet in Home as Azazel.
• Sucky Baby - Earn all Hard mode Completion Marks as Azazel.

• In the Bible, Azazel is the name given to the goat that was cast into the wild as part of Jewish atonement rituals. However, in certain traditions of Abrahamic religions, Azazel refers to the fallen angel who taught people to make weapons and jewelry and taught women the "sinful art" of painting their faces, as mentioned in the apocryphal Book of Enoch. Azazel is referenced three times in the Bible; this may be why he spawns with three Black Hearts.

• Azazel's locked character icon is a locked box with Azazel inside peeking out of it.
• He shares this locked box icon with Judas, Lilith, and Apollyon.
